Membership Savings Plan Limitations & Exclusions

The membership fee is due in full at the time initial services are rendered.
Unused benefits may not be rolled over to another membership year.
Unused benefits within the annual Church Street Dental Plan Membership are nontransferable to other plan members.
There are NO refunds.
Fees for additional services (e.g. fillings) are due at the time of service.
Fees for Prosthodontics (crowns, bridges, implant restorations, dentures) are due at the time of impression/preparation.
